Where planets are born

In the Milky Way a few hundred billion stars evolve, until they cease to be stars; but in the meantime, new stars form, and generations of stars follow. By now we have learned that many, many stars (almost all?) are accompanied, like the Sun, by planetary systems. How are these systems formed? What have we been able to understand about their nature?
